Waiver & Release of Liability

I understand that there are risks involved with my child’s participation in Oregon Premier FC competitive soccer events.

I hereby authorize the directors of Oregon Premier FC ("OPFC") to act for me according to their reasonable judgment in any emergency requiring medical attention. I hereby waive and release the directors of Oregon Premier FC from all liability and agree to accept all medical expenses incurred. I know of no physical or mental problem that will affect my child’s ability to safely participate in these sessions. I acknowledge and accept the conditions above with clicking "I agree! Let's Play" below.

I certify that my child is in good health, and may participate in strenuous physical activities. I certify that there are no physical limitations to my child’s participation in the camp. Permission is granted for my child to receive emergency medical treatment if needed. I hereby release and forever discharge Oregon Premier FC and all their agents, employees and affiliated entities from any and all liability, claims, demands, and cause of action for personal injury or death, property damage, and/or other loss suffered by my child in connection with his/her participation. I acknowledge and accept that this Release and Waiver is intended to be binding on the family, estate, heirs, executors, administrators and assigns of the minor named above. I further acknowledge and accept that this Release and Waiver is intended to be as broad and inclusive as permitted by the laws of Oregon and agree that if any portion of this release and Waiver is invalid, the remainder will continue to be in full force and effect. I agree that this Release and Waiver binds the minor and me to all of its terms.

Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19

By registering my child as a player with the Club, I agree as follows on my own behalf and behalf of my children and legal representatives:

Assumption of Risk. I understand the risks of COVID-19 and agree to assume the risk that I or my children may contract COVID-19 or another infectious disease while participating in Club activities. I understand that contracting COVID-19 or an infectious disease can lead to serious injury or death, and I agree to assume that risk. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) (and I) may be exposed to or infected by COVID-19 by attending Club activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 or another disease may result from the actions, omissions, or negligence of myself and others, the Club, employees, players, volunteers, and program participants and their families.

Full Release. I HEREBY FULLY AND UNCONDITIONALLY RELEASE, WAIVE FROM LIABILITY, HOLD HARMLESS, AND DISCHARGE FROM ALL LIABILITY THE CLUB, all agents, directors, officers, coaches, officers, managers, players, family members, employees, independent contractors, and volunteers of the Club (together, the “Released Parties”), from any and all claims, demands, costs, expenses, and actions of any nature (“Claims”) that I, my children, or our legal representatives may at any time have against any Released Person arising out of or related to contracting COVID-19 or any infectious diseases while participating in any activities associated with the Club. This release includes without limitation any Claims that we may at any time have for injury, disease, damages, death, or loss. I agree that this release will apply even if the injury, death, disease, damage or loss is caused by the negligence, fault, error or omission of any one or more of the Released Parties. I agree that this is a general release extending even to Claims that I do not know of or expect to exist when I executed this release.

Covenant Not to Sue. I agree that we will never sue or bring any legal action against any of the Released Parties for any Claims arising out of or related to contracting COVID-19 or any infectious diseases while participating in any activities associated with the Club.

COVID-19 Guidelines. I agree to follow, and to instruct my child(ren) to follow, all of the Club’s guidelines regarding COVID-19, and any social distancing guidelines issued by governmental authorities, while participating in Club activities. I understand that I or my child(ren) may be removed from Club activities – without any refund – for failure to follow these guidelines.

General. I intend this to be a full and complete release of the Club and the Released Parties to the greatest extent permissible by law. I am voluntarily registering my child with the Club and understand I am under no obligation to do so, and that this agreement is required in order for my child to participate in the Club activities. I understand that I cannot terminate or revoke this agreement for any reason. If any part of this agreement is found unenforceable, the remaining parts shall still be enforceable. I will pay the Club’s reasonable attorney fees, costs, and expenses that it incurs to enforce this agreement, including costs of any appeal. I certify by signing below that I am over 18 years of age and that I knowingly understand and agree to the terms set forth in this agreement.

I have read and understand the Oregon Premier FC Assumption of Risk and Waiver of Liability to Coronavirus/COVID-19

I hereby grant permission to Oregon Premier FC and its legal representatives, assigns, and those acting on its behalf, to use any picture, video or audio recording of my child for all manner of advertising, trade, promotion, exhibition, or any other lawful purpose related to youth soccer whatsoever and in any form or medium.

If you do not want any photos or videos of your child participating in a Oregon Premier program to be used, please email the club at info@oregonpremierfc.com with the subject matter listed as Photo Release.

I hereby release the Oregon Premier FC and the Oregon Youth Soccer Association, its member leagues, teams, agents, officers, coaches and players from all liability or responsibility for any claim, damage or legal action on behalf of the player or the player’s parents, heirs, or personal representatives, arising from any injury the player may sustain while participating in soccer or related activities, including transportation, except to the extent and in the amount covered by the OYSA accident reimbursement plan.
